Make a Referral

If you would like to access any part of our service, we have an online referral form. In the form, you can tell us what service(s) you would like to access, about your communication preferences and when you would like to meet with us. 

Who can refer? 

The following people can make referrals using our form: 

  • Autistic adults aged 18+ 
  • Parents/carers/partners of autistic adults 
  • Professionals working with autistic adults e.g. health and social care professionals, services like housing support, social prescribing 

If you are a parent/carer/partner or professional making a referral on behalf of an autistic adult, you must get their consent before doing so. 

Please note that our service is for autistic adults living in the Bradford, Airedale and Wharfedale and Craven areas who get little or no funded support. To fill in the form, please click on the link below and we will be back in touch within five working days. 

To refer for support from the Health Access Project or one-to-one peer support, you must have a formal autism diagnosis. If you just want to join the mental health peer support groups, you can join if you are awaiting a diagnosis or going through the autism diagnostic process.

To make a referral, please answer the questions below and follow the instructions. 

    Do you have an Autism Diagnosis?
    Are you 18 years old or older?
    Do you receive funded support (e.g. direct payments)?

    Unfortunately at the moment we can only work with people who have a diagnosis

    Unfortunately at the moment our service is only funded to work with people who have no funded support.

    Unfortunately you must be 18 years old or older to access our service
